Under updated IRS guidelines, employers are no longer required to automatically mail Form 1095-C to all employees.
McCarthy Tire Service will provide Form 1095-C upon request. This form provides information about the health coverage offered to you during the year.

Please include your full name, last four digits of your Social Security Number, current mailing address, and the tax year requested.
Once requested, your Form 1095-C will be provided within 30 days.
Many individuals do not need Form 1095-C to file their federal tax return, but it is available if you would like a copy for your records.
